ABSTRACT:
A Pre- experimental study to assess the effectiveness of self-instruction module on knowledge regarding hormonal replacement therapy among post-menopausal women at selected urban areas in surendranagar district.
The objectives of the study were (a) To assess the pretest knowledge of women regarding Hormonal replacement therapy. (b) To evaluate the effectiveness of self-instruction module on knowledge of post-menopausal women regarding hormonal replacement therapy. (c) To associate the pre-test scores of the women with their selected demographic variables. The research approach selected for the study was quantitative research approach and pre- experimental research – one group pre-test post-test design. The study was conducted at selected urban area at Surendranagar. Total 50 samples were selected at urban areas based on non-probability convenient sampling. The researcher used structured questionnaire for data collection regarding demographic variables and pre-test. As a part of intervention self-instruction module on Hormone Replacement Therapy was carried out among post-menopausal women in the urban area. The post test score was measured with the same questionnaires after 1 week. The collected data were analyzed by using inferential statistical method. t-test was used to evaluate the effectiveness of SIM on the level of knowledge regarding hormone replacement therapy among post-menopausal women. It revealed that the mean score of pretest was 7.86 and post-test was 15. The Mean difference was 7.14. The obtained t value was 21.37. Hence it was highly significant P<0.05 level. Hence their findings of the study revealed that the Self Instruction Module was effective in improving knowledge regarding Hormone Replacement Therapy in Post-Menopausal Women of selected urban area.

INTRODUCTION:
Menopause is a natural phase in a women's life and is not a disease. It is the permanent cessation of menstruation which occurs following the loss of ovarian activity and due to a complex series of other hormonal changes. Follicle stimulating hormone is responsible for the growth of ovarian follicles during the first half of a women's menstrual cycle.1
As menopause approaches, the remaining ovarian follicles become more resistant to follicle stimulating hormone and ovaries dramatically reduce their production of estrogen and this loss of estrogen is believed to be the cause of many symptoms associated with menopause.2
An estimated one third of postmenopausal women in the India use HRT to treat symptoms of menopause and prevent chronic conditions. It has been seen that HRT users more often had better physical activity and better general health than nonusers. Women with better socio-economic status, higher education and urban population were more likely to use HRT.3
NEED FOR THE STUDY:
The menopause is emerged has an issue owing to rapid globalization, urbanization awareness of increased longevity in urban middle-aged Indian women who are evolving has a homogeneous group.4
Several epidemiological studies have shown that insomnia, depression and anxiety, and sleep disturbances are common in women during specific life phases, such has pregnancy and menopause.5
Hormone Replacement Therapy is an effective means of treating postmenopausal symptoms and preventing long term complication.
OBJECTIVES:
1. To assess the pretest knowledge of women regarding Hormonal replacement therapy
2. To evaluate the effectiveness of self-instruction module on knowledge of post-menopausal women regarding hormonal replacement therapy
3. To associate the pretest scores of the women with their selected demographic variables.
HYPOTHESES:
H1: There will be a significant different between the pretest and posttest knowledge regarding Hormonal replacement therapy among Post-Menopausal women in selected Urban area.
H2: There will be significant association between pretest level of knowledge regarding Hormonal replacement therapy among Post-Menopausal women and the selected demographic variables.
MATERIAL AND METHODOLOGY:
RESEARCH APPROACH:
Quantitative research approach.
RESEARCH DESIGN:
pre- experimental one group pre-test post-test design.
RESEARCH SETTING:
The study will be conducted in selected Urban area of Surendranagar district.
VARIABLES:
Independent variable:
Independent variables in this study are demographic variables such as Mother’s Age in years, Marital status, Religion, Number of children, Education, Occupation, Type of work, Monthly income and previous knowledge.
Dependant variable:
Level of knowledge of the Post-menopausal women regarding Hormonal Replacement Therapy.
POPULATION:
Target population:
Post Menopausal women in Surendranagar district.
Accessible population:
Post Menopausal women who are available in selecting Urban area of Surendranagar district.
SAMPLE SIZE:
The sample size were 50.
CRITERIA FOR SAMPLE SELECTION:
Inclusion Criteria:
1. Women who are post-menopausal women in selected urban area of Surendranagar.
2. Women who are willing to participate in this study.
Exclusion Criteria:
1. Whomen who cannot read and understand Gujarati language.
2. Women who are not present during the data collection period.
SAMPLING TECHNIQUE:
Non Probability Convenient Sampling technique had been used to select the sample.
MAJOR FINDINGS:
1. Majority 19 (38%) of the respondents were 45-48 years old.
2. Majority 46 (92%) of the respondents were Hindu religion.
3. Majority 30 (60%) of the respondents had two child.
4. Majority 24 (48%) of the respondents had up to Primary education.
5. Majority 38 (76%) of the respondents were housewife.
6. Majority 46 (92%) of the respondents were Married.
7. Majority 15 (30%) of the respondents had monthly income Rs.5000-10000 per month.
8. Majority 29 (58%) of the respondent were Joint family.
9. Majority 26 (52%) of the respondent had previous knowledge related to HRT.
With regard to knowledge score pre-test mean score was 7.86, with S.D of 2.62, and the post-test mean score was 15 with S.D. 2.76, with a mean difference of 7.14 and the calculated ‘t’ value was t=21.37 which was greater than the table value. Hence, there was a high statistical significance at p<0.001 in SIM for knowledge score.
CONCLUSION:
The main conclusion from this present study is that most of the women had inadequate and moderate level of knowledge regarding Hormonal replacement therapy in pre-test and their level of knowledge regarding Hormonal replacement therapy had improved to a great extent after self- instructional module which was revealed in post-test.
